Where do I start?

CDC, in collaboration with SHAPE America, have developed step-by-step guidance for schools and school districts to develop, implement, and evaluate comprehensive school physical activity programs.

The guide can be read and utilized by a group that either already exists (e.g., school health council or wellness committee) or a new group or committee that is made up of physical education coordinators and teachers, classroom teachers, school administrators, recess supervisors, before- and after-school program supervisors, parents, and community members.  

The Comprehensive School Physical Activity Programs Planning Templates provide your CSPAP Committee with the tools for planning all aspects of your CSPAP. The documents enable you to enter information and save your work on an ongoing basis and continually add information. Refer back to the Guide as you move through each of the steps for development.

  • Read through the guide so that you understand the process and how it will support the development, implementation and evaluation of a CSPAP in your school.
  • Download CSPAP Checklist to record your actions throughout each step.
  • Download Table 1. CSPAP Committee Members and Their Roles to list the names of the people included in your school CSPAP Committee. Identify an individual who will serve as the physical activity leader.
  • Pages 18-20 in Comprehensive School Physical Activity Programs: A Guide for Schools will guide you through this process.

  • Use the CSPAP Checklist to record your actions during this step.
  • Decide on a tool or tools from the list below to conduct a needs assessment and identify what is currently in place at your school.

Throughout the needs assessment process, identify the strengths of your existing program, the gaps and corresponding opportunities that you identify throughout the process, and the priorities to consider as you develop your program. Use this chart to collect your ideas for discussion with the CSPAP Committee.

Assessment Tools

These assessment tools can be used by a CSPAP committee to gather information that will enable the committee to develop goals, objectives, and identify and plan activities for the CSPAP. It is up to the CSPAP committee to determine which tool/s to use.

  • Use Table 2. Template for Vision Statement, Goals, and Objectives to record the school vision, goals and objectives that your committee develops.
  • Review your entries in Table 1A - Identify Strengths, Opportunities, and Priorities worksheet. Work with your committee to brainstorm ideas for developing your school vision, and the goals and objectives to accomplish it. Pages 22-27 of Comprehensive School Physical Activity Programs: A Guide for Schools will guide you through this process.
  • The resources and tutorials below can assist you in learning more about writing good goals and SMART objectives.
Guidance Documents

CDC has created evaluation briefs and tutorials that guide users through the process of developing goals and objectives, enabling user s to explore the process with supportive step-by-step resources.

  • Plan the activities that will be part of your CSPAP. Pages 30-40 in Comprehensive School Physical Activity Programs: A Guide for Schools will guide you through this process and provide suggestions for decisions you will make.
  • Download Table 3 - Questions to Identify School Resources to brainstorm the assets, material resources and human resources currently available in your school.
  • Download Table 4 - Planning Chart for CSPAP Program to identify each of the activities that you will include in your CSPAP plan based on the identified goals and objectives.
  • Download Table 5 - Template for CSPAP Budget to identify the funding needed for your CSPAP activities and determine how you will secure that funding.

  • Download Table 6. Template for CSPAP Implementation Plan to develop the action planning steps of who will be responsible for which activities, tasks, and other identified steps. All of the information will guide the actions of your committee as you implement the plan in your school, and can be modified at any stage to include new tasks.
